This comparison is based on published trial data, Novo Nordisk's public announcements, and the licensed Wegovy injection. Wegovy injection has been one of the headline weight-loss medicines in the UK for the past few years. The Wegovy pill — once-daily, oral, and using the same active ingredient — gives eligible adults another way to consider semaglutide treatment. If you are considering either, the question is reasonable: same molecule, different delivery, which one suits you?
Here is the short answer, then the long one.
The active ingredient is the same — semaglutide. Trial efficacy is broadly in the same league. The differences that matter most for patients are practical: once-daily versus once-weekly, oral versus subcutaneous, room-temperature tablet storage versus refrigerated pen storage before first use, dosing rules versus a simpler weekly injection routine.
If you have a strong needle aversion, travel frequently, or struggle with weekly-injection routines, the pill may be the better fit. If you would rather take a medicine once and forget about it for six days, the injection may suit you better.
Both options are prescription-only medicines. The right choice depends on your medical history, lifestyle, treatment goals and clinical suitability.

Semaglutide is a peptide — a small protein. Peptides are normally broken down by the digestive system, which is why many peptide medicines are given by injection. The Wegovy pill is made absorbable through the stomach using tablet technology that allows enough semaglutide to survive and enter the bloodstream to be clinically useful.
Both formulations contain the same molecule. Both bind to the same GLP-1 receptor, with the same downstream effects on appetite, gastric emptying and glycaemic control. What differs is how the molecule gets into the body.

Wegovy injection's pivotal STEP-1 trial showed mean weight loss of approximately 14.9% at 68 weeks in adults with obesity, compared with 2.4% with placebo. In the same trial, 86% of participants taking semaglutide reached at least 5% weight loss.
Oral high-dose semaglutide's OASIS-4 trial studied once-daily oral semaglutide 25 mg over 64 weeks. In the treatment-policy analysis, participants taking oral semaglutide lost an estimated 13.6% of body weight, compared with 2.2% with placebo. When treatment was adhered to, Novo Nordisk reported mean weight loss of 16.6%.
Two notes of caution matter before you compare the numbers. First, the trials enrolled different populations under different protocols, so direct numerical comparison can overstate the precision. Second, real-world weight loss may be lower than trial averages because real life involves missed doses, changes in routine, side effects, and other events that can disrupt treatment.
The honest summary: the two formulations appear, on current evidence, to be in the same efficacy league. Neither is clearly better than the other on weight loss alone.
Wegovy injection: once a week, by subcutaneous injection. The standard injection schedule gradually increases from a low starting dose to a maintenance dose, under clinical supervision.
Wegovy pill: once a day, by mouth. The UK tablet has been approved for eligible adults, with dosing and escalation to be followed according to the official product information and prescriber instructions.
Critically — and this is the most under-appreciated practical difference — the pill must be taken on an empty stomach, with a small amount of water, before food, other drinks or other oral medication. This is not a polite suggestion. The pill's absorption depends on the stomach being effectively empty. People who cannot or will not follow that routine may reduce how well the medicine works.
The injection has no equivalent empty-stomach dosing rule. It is taken once weekly, with or without food, at a time agreed with your prescriber.
Both formulations share the GLP-1 class side-effect profile: nausea, vomiting, diarrhoea, constipation and abdominal discomfort. Most side effects are mild to moderate and often improve as the body adapts.
Two profile differences are worth noting. The pill is taken every day rather than every week, which may make early nausea feel different from the injection's weekly rhythm. The pill's strict empty-stomach requirement also means that taking it incorrectly may reduce absorption and affect tolerability.
The injection can also cause injection-site reactions such as redness, itching or a small lump. The pill avoids injection-site reactions, but it has stricter administration rules.

Storage: Wegovy injection pens usually require refrigeration before first use. The Wegovy pill is a tablet and is expected to be easier to store, subject to the official product information.
Travel: Injection pens may require planning, especially for longer travel or warmer temperatures. Tablets are generally easier to pack and carry.
Privacy: Some patients find the discretion of a daily pill preferable to a weekly injection routine.
Discomfort: Injection sites can develop redness, itching or a small lump. The pill has no injection site.
Needle anxiety: For some patients this is the deciding factor on its own. Wegovy injection uses a thin, fixed needle in a pen, and many users report minimal discomfort. But minimal is not none, and once a week is still 52 injections a year.
